Univita's Re-Admission Reduction Solution Expands to Central Florida


Univita Health, the leader in post-acute and home healthcare solutions, recently expanded its proprietary TeleHealth-backed Re-admission Reduction Solution and Hospital to Home programs in the Orlando metro area and surrounding communities in partnership with a major health plan.

Both programs are administered by specially-trained registered nurses and are centered on the patient, caregiver and primary care physician. Utilizing this approach, our programs have significantly reduced hospital re-admissions among high-risk patients with conditions such as chronic heart failure, COPD, diabetes, hypertension and chronic kidney disease.

Univita's proprietary Re-admission Reduction Solution is built on clinical algorithms that utilize cutting-edge, noninvasive technology to monitor high-risk patients. The program also includes comprehensive patient assessments, daily biometric monitoring and frequent telephonic educational "health coaching" sessions. When issues arise, they are immediately flagged and reported to the patient's primary care physician or specialist for immediate intervention.

"A study based on past Re-admission Reduction Solution results showed that chronically ill patients on the program experienced an 81.8% reduction in re-admissions," said Victor Valdes, M.D., and Executive Vice President of Clinical Innovation at Univita. "We estimate that by avoiding these unnecessary re-admissions, the health plan in the study saved an estimated $4,000 annually, per patient and, more importantly, helped their members stay out of the hospital and remain in the comfort of their own home while improving their outcomes and quality of life."



The Re-admission Reduction Solution counts on a HIPAA compliant TeleHealth portal and nursing electronic medical record system for the transferring of patient-to-nurse information. We utilize FDA-approved devices that feature a "one-button" patient-friendly interface, including noninvasive single lead ECG tracing and thoracic impedance, cardiac output, stroke volume and other vital signs that provide early alerts for re-admission avoidance and allow for the gathering of import HEDIS performance measurements.

The Hospital to Home program is built on home health consultations by highly-trained registered nurses working in conjunction with case management to avoid re-admission. The program is focused on the first 30 days post-discharge, when high-risk patients are most susceptible to relapses and re-admission as determined by the Centers for Medicare and Medicaid Services.
